dc.description.abstractIntroducción: Existe evidencia en la literatura proveniente de estudios clínicos y experimentales que indica que la calidad de los injertos de los donantes renales de origen cadavérico pueden ser influenciados por los eventos que ocurren alrededor del momento de la muerte cerebral y que esto puede afectar los desenlaces postoperatorios. El objetivo de este estudio es evaluar el impacto de los factores asociados al donante y el receptor con especial énfasis en la función retardada del injerto en una cohorte homogénea de 180 pacientes trasplantados en un único centro de referencia en un período de 7 años. Metodología: Se realizó un estudio observacional, analítico, de cohorte retrospectiva de fuente secundaria de datos anonimizados; como desenlaces se evaluaron; la función retardada del injerto, el rechazo agudo del injerto, supervivencia del injerto a un año. Se realizó un análisis multivariado para determinar los factores determinantes que pudieron haber influenciado estos desenlaces. Resultados: Entre enero del 2016 y diciembre del 2022, 191 pacientes fueron sometidos a trasplante renal, de los cuales 180 fueron incluidos en el estudio, provenientes de 115 donantes cadavéricos, con una edad promedio de los donantes de 39 años, la mayoría sin criterios expandidos (85%), con un puntaje de compatibilidad medio de 8.18 puntos. La causa de muerte más frecuente es la no cerebrovascular (64,35%). Además, cerca del 48% de los donantes son multiorgánicos. Con respecto a los receptores estos estuvieron en diálisis antes del trasplante en promedio 2.84 años, siendo la hemodiálisis el método más frecuente de reemplazo renal previo; con un periodo de isquemia fría entre 12.1 y 17.6 horas y de isquemia caliente de 30.74 minutos en promedio. Con relación a la diuresis residual esta se evaluó tomando como punto de corte mayor o menor de 500 mL, 40.1% de los pacientes requirieron UCI en el postoperatorio, 7% tuvieron pérdida del injerto en el primer año y 3.3% fallo primario del injerto. Conclusión: En este estudio no se encontró que el tiempo de isquemia fría o caliente predijera los desenlaces postoperatorios del injerto en cuanto a función retardada del injerto, rechazo agudo y fallo primario del injerto, sólo una diuresis residual menor a 500 mL y el requerimiento de UCI en el postoperatorio predispusieron a una mayor función retardada del injerto. Además, se evidenció una mayor supervivencia del injerto a 1 año en pacientes con puntajes de compatibilidad por encima de los 10 puntos.spa
dc.description.abstractenglishIntroduction: There is evidence in the literature from clinical and experimental studies indicating that the quality of grafts from deceased kidney donors may be influenced by events surrounding the time of brain death, and this can affect postoperative outcomes. The aim of this study is to assess the impact of donor and recipient-associated factors, with special emphasis on delayed graft function, in a homogeneous cohort of 180 patients transplanted at a single reference center over a 7-year period. Methodology: An observational, analytical, retrospective cohort study was conducted using secondary anonymized data sources. The outcomes assessed included delayed graft function, acute graft rejection, and one-year graft survival. Multivariate analysis was performed to identify determinant factors that may have influenced these outcomes. Results: Between January 2016 and December 2022, 180 patients underwent kidney transplantation from 115 deceased donors, with an average donor age of 39 years. The majority of donors did not meet expanded criteria (85%), and the average compatibility score was 8.18 points. The most frequent cause of death was non-cerebrovascular (64.35%). Additionally, approximately 48% of donors were multiorgan donors. Regarding the recipients, they were on dialysis for an average of 2.84 years before transplantation, with hemodialysis being the most common method of renal replacement therapy. The average cold ischemia time was between 12.1 and 17.6 hours, and the warm ischemia time was an average of 30.74 minutes. Residual diuresis was assessed using a cutoff of greater or less than 500 mL, and 40.1% of patients required intensive care unit (ICU) admission postoperatively. In the first year, 7% experienced graft loss, and 3.3% had primary graft failure. Conclusion: In this study, it was not found that the cold or warm ischemia time predicted postoperative outcomes of the graft in terms of delayed graft function, acute rejection, and primary graft failure. Only a residual diuresis less than 500 mL and the need for postoperative ICU admission predisposed to a higher incidence of delayed graft function. Furthermore, a higher graft survival at 1 year was observed in patients with compatibility scores above 10 points.spa
