Introduction Keratoconus is a progressive disease characterized by corneal thinning and protrusion, which induces irregular astigmatism and leads to visual loss. Due to its remodeling capacity, the corneal epithelium may reflect early topographic changes, making it a potential marker for disease stratification. Methods An analytical cross-sectional study was conducted including 480 eyes, which were classified according to a modified version of the Amsler–Krumeich classification that considered healthy eyes as stage 0. Central (3 mm) and paracentral (3–6 mm) epithelial thickness were measured using the MS-39 tomographer, and their correlations with clinical stage were assessed using nonparametric tests, including Spearman correlations.spa
