| dc.description.abstract | El cáncer (Ca) pancreático ocupa el séptimo lugar en mortalidad global y presenta una tasa de muerte mayor al 70% en Colombia. Es el tumor gastrointestinal fuera del tubo digestivo más común, siendo considerado altamente agresivo debido a su presentación asintomática y diagnóstico tardío. Se han establecido guías de tamizaje global enfocadas a identificar factores familiares e individuos con susceptibilidad genética, sin embargo, es importante reconocer factores no modificables que permitan proponer nuevas formas de tamizaje en la población e impacten en diagnóstico temprano y mortalidad por cáncer. Por lo tanto, el objetivo de este estudio fue establecer los factores asociados a malignidad en masas pancreáticas en pacientes de una institución de IV nivel en Colombia. Estudio de corte transversal con análisis secundario de una base de datos anonimizada de pacientes con diagnóstico de masa pancreática de novo a través de ecoendoscopia en la Fundación Oftalmológica de Santander FOSCAL entre Julio de 2018 y Junio de 2023. Se analizaron características sociodemográficas, familiares, clínicas, histopatológicas y endosonográficas de los pacientes con respecto a la malignidad de la masa pancreática y se calcularon razones de prevalencia como medida de asociación. De 136 casos analizados la mediana de edad al momento del diagnóstico fue de 68 años, 57.85% de sexo femenino, la diabetes la comorbilidad más frecuente (36.03%) y solo el 5.15% de los pacientes tenían antecedentes familiares de Ca de Páncreas. El 77.2% fueron tumores malignos de cualquier índole, con el 71.3% clasificados como Ca pancreático. En relación a la prevalencia de cáncer pancreático, se encontró asociación mayor para diabetes (RP:1.3; P=0.013), consumo de bebidas alcohólicas (RP:1.33; P=0.026), masa > 3 cm (RP:1.49; p=0.012) , ictericia (RP: 1.53; p<0.001) y pérdida de peso (RP:1.53; p<0.001). La prevalencia del Ca pancreático en pacientes sin susceptibilidad genética en la región nororiental de Colombia fue elevada, por lo cual se deben plantear investigaciones futuras enfocadas en el tamizaje de esta región, estableciendo prioridad en pacientes con historia de diabetes y consumo de bebidas alcohólicas, así como estudios en pacientes que manifiesten ictericia y pérdida de peso, permitiendo un diagnóstico temprano y el ofrecimiento de un tratamiento oportuno como opción curativa. | spa |

It is the most common gastrointestinal tumor outside the digestive tract and is considered highly aggressive due to its asymptomatic presentation and late diagnosis. While global screening guidelines have been established to identify familial factors and individuals with genetic susceptibility, it is crucial to recognize non-modifiable factors that may suggest new screening modalities in the population, impacting early diagnosis and cancer mortality. Therefore, the aim of this study was to establish factors associated with malignancy in pancreatic masses among patients at a level IV institution in Colombia. This was a cross-sectional study with secondary analysis of an anonymized database of patients diagnosed with de novo pancreatic mass through endoscopic ultrasound at the Fundación Oftalmológica de Santander (FOSCAL) between July 2018 and June 2023. Sociodemographic, familial, clinical, histopathological, and endosonographic characteristics of patients were analyzed concerning pancreatic mass malignancy, and prevalence ratios were calculated as a measure of association. Out of 136 cases analyzed, the median age at diagnosis was 68 years, 57.85% were female, diabetes was the most common comorbidity (36.03%), and only 5.15% of patients had a family history of pancreatic cancer. 77.2% were malignant tumors of any kind, with 71.3% classified as pancreatic cancer. Regarding the prevalence of pancreatic cancer, a higher association was found for diabetes (PR: 1.3; P=0.013), alcohol consumption (PR: 1.33; P=0.026), mass > 3 cm (PR: 1.49; P=0.012), jaundice (PR: 1.53; P<0.001), and weight loss (PR: 1.53; P<0.001). The prevalence of pancreatic cancer in patients without genetic susceptibility in the northeastern region of Colombia was high, indicating the need for future research focused on screening in this area. Prioritizing patients with a history of diabetes and alcohol consumption, as well as those presenting with jaundice and weight loss, could facilitate early diagnosis and timely treatment as a curative option. | spa |
