Introduction: Cataract surgery has evolved in recent decades to improve visual recovery and the degree of patient satisfaction. With this, the visual results and the purpose of cataract surgery have improved. Currently there are different formulas for calculating the power of intraocular lenses. Objective: To determine the absolute prediction errors of 12 biometric formulas in eyes operated with multifocal intraocular lenses (Restor). Methodology: Secondary analysis of an observational, analytical, retrospective study. Eyes with an axial length greater than 22 mm were included, which underwent phacoemulsification with an intraocular lens with at least three months of follow-up. With the power of the lens effectively implanted, 12 biometric formulas were used. Results: The consolidated ranking order of formula performance (best to worst) was: Hill-RBF, Custom T2, T2, Custom Barrett Universal II, Panacea, Holladay 1, Barrett Universal II, SRK / T, Haigis, EVO, Custom Holladay 1, SRK / T, Hoffer Q, Custom Haigis, Custom Olsen, Olsen, Modified Olsen, UniversIOL, and Holladay 2. Conclusion: In the total group, the formula with the best performance was the Hill-RBF. In the subgroup of eyes smaller than 23 mm, the Hill-RBF formula remained in second place, behind T2, but was displaced to a fourth place in the group of eyes greater than or equal to 23 mm, in which the custom Barrett Universal II took first place.eng
