| dc.description.abstract | Comparar los resultados funcionales y evaluar las tasas de complicaciones relacionadas con el procedimiento en pacientes con oclusiones en tándem tratados con terapia endovascular, mediante colocación de stent en la arteria carótida (CAS) en el segmento cervical extracraneal de la arteria carótida interna y trombectomía endovascular (EVT) sola. Posteriormente realizar un análisis de no inferioridad frente al articulo de referencia. Materiales y métodos Estudio de cohorte retrospectivo realizado entre octubre de 2016 y junio de 2025. Se incluyeron todos los pacientes con oclusiones en tándem tratados con terapia endovascular. Los datos se obtuvieron de una base de datos clínica anonimizada del Servicio de Radiología Intervencionista de un centro de referencia de alta complejidad en Colombia. Resultados Se identificaron un total de 66 pacientes con oclusiones en tándem; 21 fueron tratados con colocación de stent carotídeo (CAS) y 45 con trombectomía endovascular (EVT) sola. Los pacientes del grupo con stent alcanzaron mayores tasas de reperfusión exitosa (TICI ≥2b: 100% vs 71%; p=0,006) y de reperfusión excelente (TICI 2c–3: 76% vs 47%; p=0,024). La independencia funcional en el seguimiento (mRS 0–2) fue más frecuente en el grupo CAS (42,9% vs 17,8%; p=0,03), con una mortalidad significativamente menor (19% vs 51%; p=0,013). La colocación extracraneal de CAS se mantuvo independientemente asociada con mayores probabilidades de reperfusión exitosa (OR ajustado: 4,98; IC 95% 1,34–18,40; p=0,016), mientras que un NIHSS basal más alto se asoció negativamente con la independencia funcional (OR ajustado por punto: 0,83; IC 95% 0,73–0,94; p=0,003). El uso de CAS se asoció con una mayor probabilidad de un buen resultado funcional, aunque sin alcanzar significancia estadística tras el ajuste (OR ajustado: 2,60; IC 95% 0,58–11,62; p=0,211). El análisis de seguridad no mostró aumento en la incidencia de hemorragia intracraneal, aunque el uso de antiagregantes plaquetarios peri procedimiento fue más frecuente en el grupo con stent. Finalmente, el análisis de no inferioridad mostró que en nuestro centro los resultados funcionales fueron menores en comparación con el reportado en el articulo de referencia de la literatura. | spa |
