| dc.description.abstract | Introducción: El trasplante de progenitores hematopoyéticos (TPH) incrementa en un 80% la sobrevida a 10 años en pacientes con enfermedades hematooncológicas y no hematológicas refractarias al manejo farmacológico. Las complicaciones pueden ser agudas (enfermedad injerto contra huésped, infecciosas, y sangrado), o tardías (incremento en el riesgo cardiovascular y alteraciones del metabolismo óseo). Su ocurrencia puede favorecer la aparición de osteoporosis (13.8 – 17%), pérdida de masa ósea en cuello femoral (9%), cadera (4.2%), columna lumbar L1L4 (2%), y fracturas (1.6%). Objetivo: Establecer el cambio en la densidad mineral ósea (DMO) en adultos sometidos a TPH alogénico un año después de la intervención y los factores clínicos y/o paraclínicos pretrasplante relacionados con este cambio. Metodología: Cohorte retrospectiva de pacientes adultos llevados a TPH alogénico entre los años 2009 y 2022 en un centro de referencia ubicado en Bucaramanga, Colombia. Se obtuvieron datos sobre la DMO a nivel de columna lumbar (L1L4) y cuello femoral, antes y un año después del trasplante. Además, información de variables paraclínicas, clínicas, farmacológicas y funcionalidad del paciente. Se estimó la prevalencia de compromiso de la DMO y se evaluó el cambio a 1 año. Se realizó un análisis univariado, una regresión lineal simple para cada variable independiente y la diferencia de la DMO, y un modelo de regresión lineal múltiple con las variables con un nivel de significancia p<0.2 en el modelo de regresión lineal simple. Estos análisis se realizaron en el programa estadístico STATA V15. Resultados: Después de tamización se incluyeron 48 pacientes que cumplieron los criterios de selección (60.4% hombres, edad media 40.6 años). Se documentó una reducción en la DMO de 4.7% en la columna lumbar y 9.8% en el cuello femoral (p<0,05) después de un año de TPH alogénico. Dos pacientes tenían osteoporosis/baja masa ósea en el cuello femoral antes del trasplante y catorce un año después (p<0.001). No se observó diferencia para la alteración en columna lumbar. Se evidenció una fractura en cuello femoral y dos en columna lumbar. Se encontró una relación negativa entre la dosis de prednisolona y la DMO del cuello femoral (p 0.007, coeficiente B -0,0086 [IC95% -0,014, 0,0025]). El modelo de regresión múltiple incluyó como variables independientes (dosis de prednisona, diabetes y recaída antes del trasplante) la dosis de prednisolona tuvo la mejor predicción y mostró un coeficiente R square 0,19, B - 0,0092 p 0,003, la Diabetes B -0,057, p 0,098 y recaída previo al trasplante B -0,038, p 0,17 Se puede decir que por cada gramo de glucocorticoide (prednisolona), disminuye la DMO en el cuello femoral en -0,9%. | spa |
| dc.description.abstractenglish | Introduction: Hematopoietic stem cell transplantation (HSCT) increases 10-year survival by 80% in patients with hematological and non-hematopoietic diseases refractory to pharmacological management. Complications can be acute (graft versus host disease, infectious, and bleeding), or late (increased cardiovascular risk and alterations in bone metabolism). Its occurrence can favor the appearance of osteoporosis (13.8 – 17%), loss of bone mass in the femoral neck (9%), hip (4.2%), lumbar spine L1L4 (2%), and fractures (1.6%). Objective: To establish the change in bone mineral density in adults undergoing allogeneic hematopoietic stem cell transplantation one year after the intervention and the pre-transplant clinical and/or paraclinical factors related to this change. Methodology: A retrospective cohort of adult patients undergoing allogeneic HSCT was assembled between the years 2009 and 2022 at a referral center located in Bucaramanga, Colombia. Data on BMD were obtained at the level of the lumbar spine (L1L4) and femoral neck, before and one year after transplant. In addition, information on paraclinical, clinical, pharmacological variables and patient functionality. The prevalence of BMD compromise was estimated and the change at 1 year was evaluated. A univariate analysis, a simple linear regression was performed for each independent variable and the difference in BMD, and a multiple linear regression model with the variables with a significance level of p<0.2 in the simple linear regression model. These analyzes were performed in the STATA V15 statistical program. Results: After screening, 48 patients who met the selection criteria were included (60.4% men, mean age 40.6 years). A reduction in BMD of 4.7% in the lumbar spine and 9.8% in the femoral neck (p<0.05) was documented after one year of allogeneic HSCT. Two patients had osteoporosis/low bone mass in the femoral neck before transplantation and fourteen one year later (p<0.001). No difference was observed for the alteration in the lumbar spine. One fracture was evident in the femoral neck and two in the lumbar spine. A negative relationship was found between prednisolone dose and femoral neck BMD (p 0.007, B coefficient -0.0086 [95% CI -0.014, 0.0025]).
