dc.description.abstractLa Preeclampsia (PE) es una enfermedad exclusiva de la gestación humana, caracterizada por aumento de la presión arterial y presencia de proteinuria. Es una de las principales causas de morbimortalidad tanto materna como fetal en todo el mundo, y en Colombia es la responsable de un alto porcentaje de fallecimientos maternos, que varía entre 28.8% - 42% (1,2). Además, la enfermedad se asocia con un aumento de cinco veces en la mortalidad perinatal. Esta patología produce un gran impacto económico y social en países en vía de desarrollo es de gran importancia, más aún si consideramos que en Colombia la mortalidad materna es 10 veces mayor a la observada en Estados Unidos (2), alcanzando una tasa de 185 muertes maternas por cada 100.000 recién nacidos vivos (1). A pesar de la importancia de la PE en términos de salud pública, los mecanismos que la desencadenan no están totalmente determinados. Se propone que la PE es una enfermedad compleja, donde la susceptibilidad genética asociada a una diversa contribución de factores medio ambientales determina la generación de una respuesta que conduce a la aparición de hipertensión, proteinuria y edemas, signos del desarrollo de la PE. Dentro de los factores genéticos postulados, está el gen de la Enzima Convertidora de Angiotensina (ECA), el cual se ha asociado a PE en algunos estudios. Se plantea que variantes polimórficas del gen llevan a un aumento en su transcripción y por ende al incremento de los niveles séricos de ECA, desencadenando vasoconstricción arteriola, disminución en la producción de óxido nítrico (NO), y alteraciones en el proceso de placentación, lo que favorecería la generación de disfunción endotelial, factor que caracteriza a esta patología. La presente investigación pretende por medio de un estudio de casos y controles, establecer sí variantes polimórficas del gen que codifica para la ECA se asocian como factor de riesgo genético para el desarrollo de PE.spa
dc.description.abstractenglishPreeclampsia (PE) is a disease exclusive to human pregnancy, characterized by increased blood pressure and the presence of proteinuria. It is one of the main causes of both maternal and fetal morbidity and mortality worldwide, and in Colombia it is responsible for a high percentage of maternal deaths, which varies between 28.8% - 42% (1,2). Additionally, the disease is associated with a five-fold increase in perinatal mortality. This pathology produces a great economic and social impact in developing countries and is of great importance, even more so if we consider that in Colombia maternal mortality is 10 times higher than that observed in the United States (2), reaching a rate of 185 deaths. maternal deaths per 100,000 live births (1). Despite the importance of PE in terms of public health, the mechanisms that trigger it are not fully determined. It is proposed that PE is a complex disease, where genetic susceptibility associated with a diverse contribution of environmental factors determines the generation of a response that leads to the appearance of hypertension, proteinuria and edema, signs of the development of PE. Among the postulated genetic factors is the Angiotensin Converting Enzyme (ACE) gene, which has been associated with PE in some studies. It is proposed that polymorphic variants of the gene lead to an increase in its transcription and therefore to an increase in serum levels of ACE, triggering arteriolar vasoconstriction, a decrease in the production of nitric oxide (NO), and alterations in the placentation process, which which would favor the generation of endothelial dysfunction, a factor that characterizes this pathology. The present investigation aims, through a case-control study, to establish whether polymorphic variants of the gene that codes for ACE are associated as a genetic risk factor for the development of PE.spa
