dc.description.abstractLa sepsis es un síndrome de respuesta inapropiada a la infección en el que hay mayor riesgo de muerte debido a la disfunción cardiovascular medida tradicionalmente por ecocardiografía transtorácica. El objetivo de este estudio es identificar el valor pronóstico de mortalidad de diferentes variables ecocardiográficas. Se realizó un estudio de cohorte prospectivo multicéntrico en Bucaramanga que estudió pacientes con diagnóstico de sepsis y choque séptico y se realizó ecocardiograma transtorácico. Se incluyeron 289 pacientes con edad promedio de 69 años, de los cuales murieron 82 durante el seguimiento a 30 días. Se encontró que la alteración del diámetro diastólico del ventrículo izquierdo, el volumen de la aurícula izquierda y el espesor de la pared posterior en diástole se correlacionan con mayor mortalidad, sin embargo la utilidad de estas variables por si mismas es controversial y consideramos que su validez pronóstica no es concluyente si no hace parte de un análisis clínico y paraclínico integral en la determinación de la mortalidad. dc.description.abstractenglishSepsis is a syndrome of inappropriate response to infection in which there is an increased risk of death due to cardiovascular dysfunction traditionally measured by transthoracic echocardiography. The objective of this study is to identify the prognostic value of mortality of different echocardiographic variables. A multicenter prospective cohort study was conducted in Bucaramanga that studied patients diagnosed with sepsis and septic shock and underwent transthoracic echocardiography. A total of 289 patients with a mean age of 69 years were included, of whom 82 died during the 30-day follow-up. It was found that the alteration of the diastolic diameter of the left ventricle, the volume of the left atrium and the thickness of the posterior wall in diastole are correlated with higher mortality, however the usefulness of these variables by themselves is controversial and we consider that their validity prognosis is not conclusive if it is not part of a comprehensive clinical and paraclinical analysis in determining mortality. More studies and different methodological approaches are required to identify a better prognostic profile of transthoracic echocardiography in sepsisspa
